Please prepare the following requirements for pick up:

1. REQUIREMENTS FOR PASSPORT APPLICATION

RENEWAL

• Old Passport
• Photocopy of 1st page, last page and latest arrival of passport if any
• 4 passport size pictures w/ collar (royal blue background )
• Thumbmark and signature upon pick up of complete set of signature
• Photocopy of all submitted documents

Note: For Renewal of follow married name (Female) attached Marriage Contact

NEW PPLICANT

• NSO Birth Certificate (if unreadable submit BC of Local Civil Registrar)
• Marriage Contract ( for female applicant)
• 4 passport size pictures w/ collar (royal blue background )
• 2 valid ID (SSS, Drivers License, PRC License, Company ID, Old School ID, ATM or Credit Card w/ Picture
• Xerox copy of all submitted documents
• Signature of applicant upon pick of complete documents

MINOR APPLICANT NEW

REQUIREMENTS - 18 years old and below

• NSO Birth Certificate (if unreadable submit BC of Local Civil Registrar)
• Marriage Contract of Parents
• 4 passport size pictures w/ collar (royal blue background )
• Passport of mother
• Signature of and thumbmark of mother (Sign in Applicant Signature & Parent Signature)
• Xerox copy of all submitted documents
• PERSONAL APPEARANCE OF MOTHER

Note : Minor applicants aged 7 below doesn’t require to appear in person, mother only

Marking’s with “by mother” should be signed and thumb marked by mother only.

Note: In case of late registration of birth, submit at least 2 additional documents:

• Baptismal Certificate
• Old NBI Clearance
• Transcript of Records
• Form 1-37
• Voter’s Registration Record

2. PROCESSING TIME

Regular filing : 17-20 working days

Rush filing      : 10-12 working days

• Day 01 of counting will start after PERSONAL APPEARANCE and not upon submission of documents to Bon Voyage International
• In case DFA ask for additional document after personal appearance, Day 01 will start upon submission of requested document

3. REMINDER

• DFA has the right to ask for additional documents.
• Fill up the undertaking form on the 2nd page
• Do not use red pen or red ink for thumbmarked
• Female applicant for personal appearance are advised to wear shirt blouse w/ sleeves
